Forms

In order to have one location on campus where academic integrity concerns are tracked, instructors of record are asked to document academic integrity concerns on the Academic Integrity Form and submit a copy to the Dean of Students Office for record keeping purposes.

As required SD BOR policy, student organizations with residences must submit a semester report indicating policy violations and subsequent consequences for their specific house.  This form at SDSMT is the Student Organization Semester Report Form.
